Canada\'s Ontario medical AI riddled with errors, audit finds
An audit found repeated serious errors in AI clinical note tools approved for use in Ontario, Canada. In simulated consultations, 12 of 20 systems recorded drug names that differed from actual prescriptions and 9 generated treatment plans not in the recordings. Seventeen systems missed key mental health information. The report also flagged flaws in evaluation criteria and the lack of mandatory physician sign-off. A health ministry spokesperson said more than 5,000 doctors participate and no patient harm has been confirmed so far.
